We exist to help people diagnosed with serious mental

illness live successfully and independently in the

community of their choice. 

We focus and specialize in community based services for people diagnosed with a serious mental illness (schizophrenia, schizoaffective disorder, bipolar disorder, and major depressive disorder with psychotic features). Our case management services follow the Assertive Community Treatment (ACT) model. Our model enables us to have a team approach to provide services to our clients in their community settings where they need assistance, rather than having services provided in a traditional office setting. Following the ACT model we engage clients to teach and develop skills in order to increase their independence and success in achieving their life goals. We work with organizations and services our clients are already engaged with in the community to provide support and blend our services to improve the success of our clients. We are able to help our clients to build on their present resources and maintain continuity, rather than starting with all new service providers. While all of our clients have some needs that are the same, each client has different life experiences that may hinder their mental health and successful independent living. We are engaged in any aspect of our clients’ lives that is having a negative effect on their mental health and success. By understanding and assisting our clients address all barriers they experience that create stressors, or trigger symptoms, we are able to assist our clients develop all the skills they need for their long term personal success.
